精细内切睑袋去除术联合眶隔脂肪移植术矫正下睑袋的疗效观察(1)
[摘要]目的：探讨应用精细内切睑袋去除术联合眶隔脂肪移植术矫正下睑袋的临床效果。方法：选取2015年1月-2017年1月在笔者医院接受下睑袋整形处理的80例患者作为研究对象，按照随机数字表法分为对照组和观察组，每组40例。对照组采用传统结膜入路下睑袋整形术去除下睑袋；观察组采用精细内切睑袋去除术联合眶隔脂肪移植术。术后7d对比分析两组患者的临床效果。结果：观察组下睑袋皮肤外观恢复时间、睑袋整形效果满意度及并发症发生率均优于对照组，差异有统计学意义(P<0.05)。结论：应用精细内切睑袋去除术联合眶隔脂肪移植术矫正下睑袋更加安全有效，具有良好的整形效果，组织破坏少，术后恢复期短，可明显提高患者满意度，值得临床推广应用。

Abstract： Objective To explore the safety and efficacy of fine inner pouch removal surgery combined with orbital fat transfer operation to the patients who accepted lower blepharoplasty. Methods 80 patients with lower blepharoplasty were regarded as the objects of research in our hospital from January 2015 to January 2017. The whole patients were divided into the control group and the observation group according to random number table method and each group has 40 patients. The control group used traditional transconjunctival approach to lower blepharoplasty，while the observation group used fine inner pouch removal surgery combined with orbital fat transfer operation.Comparison of the clinical effects of two groups of patients with 7d after operation. Results The Appearance recovery time， the cosmetic effect of blepharoplasty and the incidence of complications in the observation group were all better than those of the control group， the differences were statistically significant(P<0.05). Conclusion It is more safe and effective to use fine incision bag removal combined with orbital septum fat transplantation to correct the blepharoplasty. It has good plastic effect， less tissue damage and short recovery period， which can obviously improve patient satisfaction. It is worthy of clinical application.

眼瞼皮肤是人体最薄皮肤之一，眼睛又处于人体最显露部位，所以眼睑皮肤的老化极其影响人的外观审美且最易被察觉，结膜横切口入路下睑成形术的短期和长期并发症较肌皮瓣法少，这一观点已被普遍接受[1-2]。然而，有报道指出传统结膜横切口入路产生的问题，包括术后血肿、下睑错位、复视、囊肿和肉芽肿发生。因此，安全、副作用小且手术效果好的下睑袋整形术正日益受到重视[3]。
1 资料和方法
1.1 一般资料：选取2015年1月-2017年1月笔者医院收治的80例眼部整形患者作为研究对象，排除标准：①酗酒史、药物成瘾史者；②严重肝、肾、心功能疾病者。按照计算机数字法分为对照组和观察组，每组40例。对照组：男21例，女19例；年龄25～64岁，平均年龄(44.5±3.5)岁。观察组：男18例，女22例；年龄23～65岁，平均年龄(43.1±4.2)岁。所有患者及其家属对本次研究的内容均知情同意，并自愿参与本研究，均已签署了知情同意书。两组患者基线资料比较，差异无统计学意义(P>0.05)，具有可比性。
1.2 方法
1.2.1 对照组：采用传统结膜入路睑袋整形术去除下睑袋。具体手术方法：结膜和需要切除的脂肪组织周围行局部麻醉，选择下睑板下缘3mm处做下泪小点到外眦的切口，切开结膜和下睑缩肌，用镊子向上提起结膜和下睑缩肌同时轻压眼球后暴露眶隔脂肪，单极电刀打开眶隔，先去除中央脂肪垫然后再去除内侧和外侧脂肪垫。, http://www.100md.com(王科学 马满仓)
