生肌玉红膏治疗糖尿病足溃疡的临床疗效及对创面组织内PDGFR—α表达的影响(1)
[摘要]目的：觀察生肌玉红膏治疗糖尿病足临床疗效，及对创面组织内血小板衍生生长因子受体-α(Platelet derived growth factor receptor，PDGFR-α)的影响，为临床糖尿病足溃疡的治疗方案选择提供参考。方法：选择笔者医院2015年2月-2016年1月收治的糖尿病足患者204例，按照就诊顺序编号，采用随机数表法将其分为对照组和研究组，每组102例。两组患者均在严格控制血糖的基础上，予以清创换药治疗，研究组在对照组治疗基础上再予以生肌玉红膏辨证加减涂抹治疗。两组患者均连续治疗1个月为一个疗程。比较两组患者治疗前、治疗一疗程后血糖水平、外周血血管内皮生长因子(Vascular endothelial growth factor，VEGF)、创面肉芽组织中PDGFR-α表达变化，比较两组患者临床总有效率，及药物不良反应情况。结果：治疗后，两组患者外周血VEGF水平、创面肉芽组织中PDGFR-α均较治疗前有明显提升，且研究组患者明显高于对照组，差异有统计学意义(P<0.05)。研究组临床总有效率为92.16%明显高于对照组的78.43%，差异有统计学意义(P<0.05)。两组患者治疗过程中均未发生严重不良反应。结论：生肌玉红膏辨证加减治疗糖尿病足溃疡，可有效提升血小板衍生生长因子受体-α表达，提高临床疗效，具有较高的临床应用价值。

Abstract： Objective To observe the clinical effect of Shengji Yuhong ointment on the treatment of diabetic foot and the effect on the platelet derived growth factor receptor-α(PDGFR-α) in the wound tissue， to provide reference for the treatment options of the patients with diabetic foot. The two groups were treated with debridement and replacement therapy on the basis of strict control of blood sugar， and the study group were treated with the dialectical addition and subtraction of Shengji Yuhong ointment on the basis of the control group. Two groups of patients were treated continuously for one month as a course of treatment. The changes of blood glucose level， peripheral blood vascular endothelial growth factor (VEGF) and PDGFR-α expression in the granulation tissue of the two groups were compared before and after treatment. The total clinical effective rate of the two groups after treatment were compared， and the adverse reactions of the two groups were compared. Results After treatment， the levels of vascular endothelial growth factor in peripheral blood and PDGFR-alpha in granulation tissue of wound in the two groups were significantly higher than those before treatment， and the study group was significantly higher than the control group， the differences were statistically significant(P<0.05). The total effective rate of the study group was 92.16%， which was significantly higher than that of the control group (78.43%， P<0.05). No serious adverse reactions occurred in the two groups during the treatment. Conclusion The dialectical addition and subtraction of Shengji Yuhong ointment can effectively improve the expression of platelet derived growth factor receptor-α and improve clinical efficacy， which has high clinical value., http://www.100md.com(陈诚 吴艳 陈琴)
