富血小板血浆引导骨再生在上颌美学区单牙种植修复中的应用研究(1)
[摘要]目的：研究富血小板血浆引导骨再生在上颌美学区单牙种植修复中的应用效果。方法：选择2013年1月-2017年1月行上颌美学区单牙种植修复的225例患者为研究对象，根据是否混有富血小板血浆将其分为观察组与对照组，分别给予富血小板血浆引导骨再生、无混有富血小板血浆干预，评价各组种植修复的成功率、骨吸收、红色美学指数(PES)、白色美学指数(WES)及主观满意度情况。结果：两组均成功种植修复，两组骨吸收、WES评分比较无明显差异(P>0.05)；观察组种植修复即刻、修复后6个月、12个月PES评分及种植修复后12个月附着高度、整体美观评分均显著高于对照组，差异有统计学意义(P<0.05)。结论：富血小板血浆引导骨再生效果明显，能有效促进骨再生及修复，且美学效果良好。

Abstract：Objective To study the application effects of platelet rich plasma guided bone regeneration in single-tooth implant restoration in maxillary aesthetic area. Methods According to the use of mixed platelet rich plasma， 225 patients who underwent tooth implant restoration in maxillary aesthetic area from January 2013 to January 2017 were divided into the observation group and the control group. The two groups were given platelet rich plasma guided bone regeneration and intervention without platelet rich plasma respectively. The success rate of restoration， bone resorption， pink esthetics index (PES)， white aesthetic index (WES) and subjective satisfaction were evaluated. Results The implant restoration in the two groups was successful， and there were no significant differences in bone resorption and WES scores between the two groups (P>0.05). PES scores of the observation group at implant restoration， at 6 and 12 months after implant restoration， the attachment height and the overall appearance scores at 12 months after implant restoration were significantly higher than those in the control group (P<0.05). Conclusion Platelet rich plasma can significantly promote bone regeneration and repair， and the aesthetic effect is good.

目前，對于上颌美学区牙齿缺失，不仅要求有效修复，而且要求尽量保持义齿美观，特别是女性[1]。临床处理上颌美学区牙齿缺失的方法包括即刻种植、延期种植及骨引导再生等[2-3]，各有优缺点，考虑到牙龈形态、牙周健康维持中硬组织支持的重要性等情况，对骨量不足区建议行骨引导再生。富血小板血浆属于血小板浓缩物，生物安全性高，能单独或联合其他材料促进骨生长，对种植美学修复、组织损伤修复有重要意义[4-5]。本研究通过与未混有富血小板血浆干预比较，分析富血小板血浆引导骨再生在上颌美学区单牙种植修复中的应用价值。
1 资料和方法
1.1 临床资料：研究对象选择2013年1月-2017年1月接受上颌美学区单牙种植修复的患者。纳入标准：①均自愿要求种植修复，符合种植修复适应证；②均为上颌美学区单牙种植病例；③年龄20～65岁；④临床及随访资料完整。排除经常吸烟、重度牙周疾病、邻牙明显移位、种植修复禁忌证、相关资料不全等患者。最终入选225例，根据是否混有富血小板血浆将其分为观察组(n=122)与对照组(n=103)。观察组：男71例，女51例，年龄平均(33.65±6.48)岁；其中上中切牙79例，上侧切牙37例，上尖牙6例。对照组：男63例，女40例，年龄平均(34.00±7.02)岁；其中上中切牙68例，上侧切牙31例，上尖牙4例。对比两组性别、年龄等差异无统计学意义(P>0.05)，具有可比性。本次研究经院伦理委员会批准，患者知情并签订同意书。, 百拇医药(左胜元 徐莹)
