低剂量CT扫描技术在肺癌筛查中的应用价值(1)

     [摘要] 目的 探讨低剂量CT扫描技术在肺癌筛查中的应用价值。 方法 选取2013年5月~2014年5月240例于本院疑诊或确诊为肺癌的受检者240例,将其随机平均分为3组,均行胸部CT扫描。A组使用CARE Dose 4D技术,参考管电流设置为100 mAs,其余两组不使用CARE Dose 4D技术,管电流分别设置为100(B组)、50 mAs(C组)。比较3组的容积CT剂量指数(CTDIvol)、有效辐射剂量(ED)、图像噪声、图像质量评分及冠状动脉钙化灶数量。 结果 与 C组比较,A、B组的CTDIvol、ED明显增加,而其图像噪声、图像质量评分和钙化灶数量方面明显优于C组(P<0.05);A组的CTDIvol、ED明显低于B组,但图像噪声、图像质量评分、钙化灶数量差异无统计学意义(P>0.05)。 结论 CARE Dose 4D技术能够在保证CT诊断效果的基础上降低辐射剂量,值得推广。

    Application value of low-dose CT scanning technology in screening of lung cancer

    [Abstract] Objective To investigate the application value of low-dose CT scanning technology in screening of lung cancer. Methods 240 subjects suspected or diagnosed with lung cancer in our hospital from May 2013 to May 2014 were selected and they were randomly and evenly divided into 3 groups.All subjects were given CT scanning of chest.CARE Dose 4D technology was used in group A,and the reference tube current was set to 100 mAs,while CARE Dose 4D technology was used in the other two groups,tube current was set to 100 mAs (group B) and 50 mAs (group C) respectively.The volume CT dose index (CTDIvol),the effective dose (ED),image noise,image quality score and quantity of coronary artery calcification focus among 3 groups was compared respectively. Results Compared with group C,the CTDIvol and ED of group A and group B was obviously increased,while the image noise,image quality score and quantity of coronary artery calcification focus in group A and group B was better than that of group C respectively (P<0.05);the CTDIvol and ED of group A was obviously lower than that of group B,while there was no statistical difference of image noise,image quality score and quantity of coronary artery calcification focus between group A and group B (P>0.05). Conclusion CARE Dose 4D technology can reduce the radiation dose on the basis of guaranteeing diagnostic effect.It is worthy of promotion.

    随着CT检查在临床中被日益广泛应用,医疗辐射造成的危害也被人们日益重视。国际放射防护委员会指出:每增加1 mSv的X线照射剂量,则增高5/105的概率引发肿瘤[1],所以遵循ALARA原则在保证图像质量的前提下降低辐射剂量已成为现今CT技术研究的重点。鉴于年龄>55岁、精神压力及吸烟均为肺癌、冠心病共同的高危因素,本研究对来本院疑诊或确诊肺癌的240例患者应用CT技术行肺癌筛查及冠状动脉钙化灶情况普查,并总结不同曝光条件控制下受检者所受辐射剂量、图像质量及对其临床诊断效果的影响和意义。
